I am aware that Couche Nail Bar uses a variety of products and tools during treatments, including nail polishes, polish removers, files, scissors and other utensils. Recognize the importance of providing accurate information about my health, including allergies, skin conditions, infections or previous injuries, which could affect the provision of services in the salon. I understand that it is my responsibility to inform salon staff of any relevant medical conditions prior to receiving any treatment.

Consent for Images:

I agree that photographs of my hands, feet or treatment results may be taken for Couche Nail Bar promotional or marketing purposes, as long as my privacy is respected and my explicit consent is requested before any image that includes me is posted.
